    Hi,

I am a new member here and was hoping to get some advice on orthotics. I have been wearing custom made NHS precsribed orthotics for about 10yrs. However, I feel that they are not effective as in this time I've developed chondromalacia patella in both knees. My shin pain and hip pain did reduce significantly.

I have reading that orthotics made using collapsible foam (standard NHS procedure) is not ideal and plaster casting is the best method.

I was hoping somebody could recommend me an experienced podiatrist who specialises in plaster casting in London. I'd appreciate your advice, many thanks.

I would suggest that one not focus on the particular method by which a cast of the foot is made, and more on the skill of the doctor in assessing the condition, the diagnostical basis and need for orthotic and the prescription incorporated in them. Feet do change over the years, and the need for a new prescription for orthotics would be common as those changes occur. In my experience, ten years would be a very long time for any patient to be wearing a single pair of orthotics and to have them continue to function well, if, in fact, they ever did. What expertise do you possess to conclude that the orthotics play a part in your patella problems, and for what diagnosis (not symptoms) were the orthotics initially prescribed? If you cannot definitively answer those questions, and you probably can't, then this whole discussion is unproductive at its center.
