For years, there seemed to be a little tingle whenever I touched the top of the bottom joint of my big toe. Now, I have to be careful of wearing certain shoes or flipflops. It looks like a little raised area, shiny on the top and when I touch it, it feels uncomfortable, like a tingle or electric shock. What is this? I can bend my toe, but I cannot touch the area.

I really can't picture what you mean by "the top of the bottom joint of my big toe," but it would seems that you have a disorder of the sensory nerve itself, or that something is creating irritation of or pressure on it, due either to some sort of soft tissue or bony mass or perhaps an entrapment. At any rate, it would seem more than likely that the problem is local to the area of the symptoms rather than remote. Just what it might be can only be evaluated by direct examination and perhaps nerve testing. See a good podiatrist in your area.
