very sharp pain on top of foot, hard lump

    My 54-year old brother has a very sharp pain on the top of his foot over his arch when what appears to be a bone spur is touched. He has high arches and the lump is it is about pea-sized and hard. When touched it "sends him through the ceiling." The rheumatologist didn't see anything wrong on x-rays. What would cause this? His hobby is fencing and it is the foot that is presented forward. Also, how to fix before he runs through his pain meds. Any advice is gratefully accepted.
