Toe Infection?

Some background. I live on a small island in a foreign country with one doctor who cannot communicate fluently in english, but we can get by and occasionally I can go in with a translator.

Last december a table fell on my right big toe and I had to have the nail removed. It seemed to heal just fine, then sometime in december I was playing volleyball and experienced some pretty intense pain. A close look at my toe and it was puffy, swollen, and red around the nail.

The local doctor prescribed 5 days of penicillin for the infection. There was no change, so he prescribed another five days of penicillin. Still no change so he prescribed another five days of penicillin. At this point the doctor decided penicillin wasn't going to fix it, so he prescribed me five days of cephalexin. This didn't seem to work either. We decided I should book a flight to the big city to go to a hospital.

There was about a two week period between this visit and my flight. In this time I came down with a cold and had a fever for about six days. This thankfully went away before my flight.

I then flew for the nearest large city and saw an english speaking surgeon who said it was an ingrown toenail. He cut back the toenail and dug out the infected areas. And gave me some painkiller nad three days worth of cephalexin.

I'm now back on my island. it's been a week since the surgeon removed the toenail. The areas he removed seem to be growing back, and I can walk normally without too much pain. However , the toe remains red and swolen and I've had a mild fever(98.9 - 99.15) every day since, also my entire right foot is very warm to the touch while my left foot can be quite cold to the touch. The doctor on the island only changes the bandages and really has no comment on the fever or local heat in the foot. I'm really concerned because of the duration of the infection and the doctor admits there might still be a mild infection, but he suggests no remedy. I'm in serious need of advice. Any would be helpful.

This is not something that can be dealt with in a forum. One way or another, you need knowledgeable hands-on evaluation and care or the consequences could be very drastic with life and limb potentially at stake. A prompt trip back to the surgeon might be well worth the time, cost and trouble. Infections are nothing to be dallied with, and prompt attention may be critical.
