External fixators are certainly acceptable in treating what appears to be a comminuted (multiple pieces) fracture of your 1st metatarsal. They are great to use when the bone pieces are so small and/or damaged that the surgeon is unable to drive screws through them in order to reduce you fracture. The cement is of course surgical grade bone cement not the kind you use to build your basement. It is certainly appropriate and safe to use as a bone substitute (to fill any defects left behind after the surgeon cleaned the fracture site) but also, as a rigid surface (an anchor, if you will) for the external fixator to be stabilized on so that your fracture can heal. 10 weeks minimum, if you want to avoid surprizes
