Any hint or guess?

    Ok it's really ridiculous but my friend has this on his leg for couple years. My wild guess is that he overworked his legs with time - standing all the time at work and is kinda old today - 57 years.
    Sure he has to go to doctor for accurate tests. But maybe someone knows what this thing could be?
