some variant of Joplin neuralgia

    I am having right medial great toe neuralgia/likely some variant of Joplin neuralgia for 13 years, with a digital branch of the superficial peroneal nerve as the source of the pain. Sensitive to touch the toe pain area and severe needle pain occurs randomly without any touch. I did MRI, CT and X-ray scans but no sign of damage. I had below procedures and no effect on pain.
    1. RFA twice
    2. PRP injections
    3. Cortisone and Kenalog injections
    4. Laser therapy
    Took medications like Cymbalta and Gabapentin but doesn't help the pain. I have to use wide footwear to avoid touching the toe pain area and used orthodontics insole but no help.
    Please help.

    A Joplin's neuroma is typically caused by high pressure under the big toe when walking. When there is high pressure under the toe the sock liner of the shoe will show much more wear under the big toe compared to anywhere else. To reduce pressure on the big toe use a shoe with a rocker tip. Most running shoes have a rocker tip. Some have a lot more rocker than others. Rocker tip = thickness of midsole of the shoe is much greater just behind the toes than it is at the tip of the toes.
